PROBLEM TO BE SOLVED: To enhance the reliability of a monitoring circuit for the input-side line and the output-side line of an AD-DC converter and suppress its cost. SOLUTION: A power supply device 10 includes the AC-DC converter 1 that converts an input AC voltage into a DC voltage and outputs it. In this power supply device, one input-side line 1a1 and the other input-side line 1a2 of the AC-DC converter 1 are connected through the normally open contact RY1L of a first relay and the coil RY2M of a second relay connected in series. The second normally open contact RY2L of the second relay is connected in parallel with the normally open contact RY1L of the first relay. One output-side line 1b1 and the other output-side line 1b2 of the AC-DC converter 1 are connected through the coil RY1M of the first relay. Part of a failure signal output circuit 14 is formed of the normally closed contact RY1R of the first relay and the first normally open contact RY2R of the second relay connected in series.
