PURPOSE: A chip on film with an identification code, an apparatus for forming the identification code and an inspection apparatus including the apparatus are provided to improve the reliability of a punching and a separation processes by identifying the location of good or bad chip unit. CONSTITUTION: At least one chip unit(30) is arranged on a base film(10). Puffer holes(11) are formed with a constant interval in order to transfer the base film. An identification code(20) is printed on the base film. The identification code identifies the location of at least one chip unit. The identification code is formed between the region in which at least one chip unit is arranged and the puffer holes.
