PROBLEM TO BE SOLVED: To provide an image forming apparatus that sets the optimum image forming conditions to meet an operator's request. SOLUTION: The image forming apparatus makes test printing where a plurality of patch images 93 to 99 having same density are transferred onto one sheet by switching transfer voltage to a different value for each patch image and then outputted. Images showing level values 92 indexing transfer voltage values used to transfer the patch images 93 to 99 are formed together with the patch images for each patch image on the output test patten. An operator views the output test pattern, and inputs and sets the level value corresponding to the patch image considered as the one having optimum image quality by himself (herself) as the transfer voltage value used when forming an image from a setting input receiving screen displayed on the control panel of the image forming apparatus.
