FIELD: medicine. ^ SUBSTANCE: partial ileoshunting is performed. Access to place of intervention and application of anastomosis is performed by means of endovideosurgical technology. Into abdominal cavity through skin cut and trocar above navel laparoscope with connected to it videocamera is introduced. Under visual control through two trocars above lap and on the right in mesigastrium on mid-clavicular line instruments are introduced. Ileocecal angle is visualised. Excluded part of ileum is measured from it in portions between two Veresh's needles. Adducting and abducting parts are marked. Hole in mesentery is made and thread-holder is passed through it. Intestine is transected, parts are closed. Adducting part is brought by thread to terminal part of ileum and fixed isoperistaltically. Application of small-large intestinal anastomosis is performed. In special cases anastomosis is applied laparoscopically. Intestine is transected by apparatus. For application of anastomosis apparatus is used. In special case anastomosis is applied laparoscopically in assisted way. During transaction intestine is extracted from abdominal cavity. Small-large intestinal anastomosis is applied by two-layer suture. ^ EFFECT: method allows to minimise operation trauma, realise early activation of patient's, reduce number of complications, financial expenditures, improve cosmetic effect. ^ 3 cl, 2 ex |
