<P>PROBLEM TO BE SOLVED: To protect personal information, and to prevent erroneous delivery with regard to an information concealing form. <P>SOLUTION: An upper form 4 is laid so as to be pressure-bonded on a lower form 3 having a heat-sensitive color-developing layer 12 through a pressure sensitive adhesive layer 16 to be overlapped with each other to form a concealed information column 21. Information not to be concealed is printed only on a part of the lower form 3 which is not covered with the upper form 4, and information to be concealed is printed on the heat-sensitive color-developing layer 12 from above the upper form 4. The upper form 4 is adhered to the lower form 3 through the pressure sensitive adhesive layer 16 by applying pressure after printing to conceal the information printed on the concealed information column 21. <P>COPYRIGHT: (C)2010,JPO&INPIT |
