<P>PROBLEM TO BE SOLVED: To provide a leaflet with a supplement, including two front sheets and one intermediate sheet bonded at one end edge and a seal part formed in the other end edge of the intermediate sheet to be unsealed, and also to provide its manufacturing method. Ž<P>SOLUTION: The method for forming the leaflet with a supplement includes the steps of: performing printing on a wide sheet and a narrow sheet larger by a predetermined size than 1/2 of the width of the side sheet with equal picture pitches; forming a perforation in a predetermined position of the printed narrow sheet; forming a slit in the center of the printed wide sheet in a width direction; aligning and stacking edges of the narrow sheet and the wide sheet to bond the sheets by one edge; folding the bonded wide and narrow sheets in the center of the width direction with the narrow sheet inside; bonding the other edges of the wide and narrow sheets to the opposite surface; and cutting the sheets in a predetermined position.
