The instrument i.e. surgical sighting ancillary (10), has a body defining a cylindrical through hole (12) and receiving a guiding sleeve (14). Blocking units block translation of the sleeve along a rectilinear axis (X-X), and comprise an elastically deformable unit (18) elastically deformable between a configuration in which the deformable unit partially closes the hole and another configuration in which the deformable unit does not close the hole. The sleeve controls passage of the deformable unit between the configurations by rotation around the axis.
