PROBLEM TO BE SOLVED: To provide an optical transmission module free from the occurrence of skew between respective optical signals. SOLUTION: In the optical transmission module 1 which includes: a plurality of optical elements 3 which are mounted on an optical waveguide 2 in parallel or are mounted in opposition to an end face of the optical waveguide and transmit or receive optical signals; a lens 4 provided at an end part of the optical waveguide 2; and a plurality of optical wires 5a to 5d formed between respective optical elements 3 and the lens 4, the optical wires 5a to 5d are crossed each other so that the optical wires 5a to 5d have the same optical path length.
