PROBLEM TO BE SOLVED: To provide a die processing apparatus for a light guide member forming apparatus that easily process a tip shape of a rod-like die, to provide a light guide member forming apparatus equipped with a template formed by the die processing apparatus for the light guide member forming apparatus, and to provide a light guide member formed by the light guide member forming apparatus.

SOLUTION: A lower template 21 of the light guide member forming apparatus is provided with a recessed portion 211 as a cavity for resin-molding the light guide member guiding light from a light incident surface to a light projection surface. The die processing apparatus 10 is provided with the rod-like die 11 for processing the recessed portion 211. The rod-like die 11 has its tip formed into a nearly cylindrical body having a convex surface 111 corresponding to the light projection surface, and also has both sides of a center axis L cut to form a parallel plane portion 112 along the center axis L.
