<P>PROBLEM TO BE SOLVED: To provide a postcard inclusion form capable of protecting surely transmission information to form a postcard and to transmit it, by manufacturing the postcard inclusion form using a paper roll, by forming a roll-bent form having 18 inches or more of width, and a described information protection seal part in a form piece adjacent to the postcard, and by folding it in a folding part in a boundary. Ž<P>SOLUTION: This postcard inclusion form comprises the first sheet adjacently connected with the plurality of form paper pieces including the postcard in the folding part, and the second sheet having a height same to that of the first sheet, and joined to be overlapped with the first sheet in a folding-back part of a width-directional paper end of the first sheet, a pressure-sensitive adhesive paper sheet comprising a transparent base paper sheet via a pseudo-adhesive structure part is bonded under a condition of exposing a separation paper sheet, in a postcard transmission information describing face side of the form paper piece adjacent to the postcard of the first sheet.
