A photoablator (1) for perforating a nail, having a laser source (2), optics (3) for focusing a laser beam emitted by the laser source (2) in a target plane, and beam positioning means (4) for positioning the laser beam in relation to the nail. The photoablator (1) further comprises a treatment area definition unit (13) for automatically defining an area of the nail to perforate by means of the laser beam. By means of the treatment area definition unit (13) the nail of a target finger or toe of a patient can be perforated respecting the specific conditions of the nail. Taking into account these conditions, the treatment area including a suitable treatment microhole pattern can be quickly defined by the treatment area definition unit (13), wherein according computing means can be used for this purpose.
