A method for the surgical treatment of the destructive pancreatitis provides for the opening of all fluid-containing retroperitoneal masses, the evacuation of pyo-necrotic content, and the elimination of the sequesters under videoendoscopic control. Mini-invasive lumboscopic intervention is provided under the permanent videoendoscopic control using EndoTIP port (Carl Storz) in diameter and end-face laparoscope. The site of laparoscope insertion locates at the edge of middle and upper thirds of the distance between the rib XII and wing of ilium in the middle between median and posterior axillary lines. The skin is cut, 1.0- long, and the laparoscope is passed referring to the point in the middle of the distance between the external edges of the body in the imaginary line drawn sagittally through the apex of xiphoid. At the same time, carbon dioxide is insufflated. To the parapancreatic fat, the silicone draining tubes are installed in internal diameter with lateral perforations. The repeated revision of the cavity are performed with the aid of videolaparosope inserted via the draining tube. |
