<P>PROBLEM TO BE SOLVED: To solve the problem that a conventional booklet with a duplicating form needs a broad table when it is used and tends to give a vague idea about its practical usage which is peculiar, although it is of such a type as not requiring the insertion of an underlay for smooth writing etc. during entering information. Ž<P>SOLUTION: A booklet with a duplicating form is constituted of a plurality of different kinds of duplicating form parts bound at one end of a single booklet.
