SOLUTION: The image output apparatus includes a determination means for determining whether the density of a document image exceeds a threshold, and a control means for inhibiting the composition and output of a two-dimensional bar code and the document image if the determination means determines that the density exceeds the threshold and combining and outputting the two-dimensional bar code and the document image if the determination unit determines that the density does not exceed the threshold.
