<P>PROBLEM TO BE SOLVED: To provide a concavo-convex forming device in which variations in height position of a convex portion can be suppressed, and to provide an information input device. Ž<P>SOLUTION: The concavo-convex forming part (concavo-convex forming device) 11 is provided with a projection member 60 which protrudes from an initial position O to a prescribed position P by being pushed up from the bottom side and forms input projections 1-5, a driving part 41 which is displaced capable of ascending and descending and pushes up the projection member 60, and a side wall 52 which regulates the projection member 60 so as be located at the prescribed position P by engaging with the projection member 60 at the time of projection of the projection member 60.
