A circuit board for collecting a body fluid comprises a first substrate having a lancet needle, a second substrate so disposed as to have a space in relative to the first substrate on the upstream side of the lancet needle in the puncturing direction, a flexible insulation layer installed between the first substrate and the second substrate, and a conductor pattern which integrally has an electrode, a terminal and wiring for electrically connecting the electrode and the terminal, and is provided on the flexible insulation layer.
