<p><P>PROBLEM TO BE SOLVED: To provide a laminate sheet for delivery, which is easily broken by hand, in the case of a breakable loaded body, with a base material sheet left on the loaded body, and a delivery slip using the same. <P>SOLUTION: The laminate sheet 10 for delivery is provided with a display sheet 3 via a thermoplastic resin layer 2 in one surface of a fragile base material sheet 1, and an adhesive layer 4 in the other surface.
