<P>PROBLEM TO BE SOLVED: To provide a declaration form information management system securely and inexpensively managing data and easily and accurately converting information described in the declaration form into data. <P>SOLUTION: Declaration contents are input through a web screen, a server computer generates-returns printing data of a declaration form on the basis of input data, and a declarer prints the declaration form. An area Ta for displaying input data in characters and an area Tb for displaying the input data in a two-dimensional code 4 are set in the declaration form, the two-dimensional code 4 is read and converted into data at declaration, and the data is recorded and stored in a data managing device.
