<P>PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of printing original data corresponding to a print document faster than before. <P>SOLUTION: A data management part 111 preserves print data 51 having a plurality of page data in a file server 3 as original data 52 and prepares the preservation destination information 56 of the original data 52. Composite print data 53 for which a two-dimensional code 55 corresponding to the preservation destination information 56 and the page data 511 corresponding to the first page are printed and output as the print document 60. A scanner part 14 reads the print paper sheet 61 of the print document 60 and prepares image data 54. A code processing part 112 extracts the two-dimensional code 55 from the image data 54 and acquires the preservation destination information 57 of the original data 52.
