<p><P>PROBLEM TO BE SOLVED: To provide an improved questionnaire execution and collection system, enabling a user to reply freely, making easy collection of a greater number of questionnaire response results, and being capable of reducing a load on the questionnaire execution side. <P>SOLUTION: A questionnaire system 1 includes a server 101 and radio communication terminals 105-1 to 105-6 communicating with the server 101 via a communication network 111. The server 101 stores questionnaire contents, and a schedule by which each radio communication terminal is to execute the questionnaire contents. The radio communication terminal acquires the schedule and the questionnaire contents related to the self by accessing the server 101 via the communication network 111, stores the acquired schedule and the questionnaire contents, executes the corresponding questionnaire contents based on the schedule, and transmits the obtained reply to the server 101.
