A foam sheet is provided to prevent functional deterioration of the sheet while facilitating cutting of the sheet by heat-treating cutting patterns on the surface of the sheet only, and an apparatus for manufacturing the foam sheet is provided. An apparatus(200) for forming cutting patterns comprises a forming roll(201) and a cooling roll(202) which are brought into contact with each other and installed on a top part of a rectangular opened frame(300). The frame has a flame roll(203) installed in an inner part thereof, and the flame roll is mounted on a moving device(204) capable of being moved vertically and horizontally. The frame has guiding rolls(11-15) installed on an outer part thereof to supply the foam sheet to the forming roll on the top part of the frame after moving a foam sheet supplied from one side through right side, bottom and left side of the frame. A polyethylene foam sheet(100) is guided to a draw-off roll after sequentially passing through the guiding rolls, forming roll and cooling roll from a feeding roll. The foam sheet is heated through the flame roll and has latticed patterns formed on the surface thereof through the cooling roll and forming roll.
