PROBLEM TO BE SOLVED: To provide a device for forming a laser beam. SOLUTION: This device is provided for forming the laser beam having a partial beam 3 emitted from a laser diode bar 1 for forming the laser beam mutually separated in the vertical first direction X to the progress direction Z of the laser beam. The device has a first reflection interface 8, that is, a first reflection interface capable of differently diffracting at least a plurality of partial beams 3 of the laser beam formed so as to advance by being mutually further converged at least partially after passing through the first reflection interface 8 rather than before passing through the first reflection interface 8, and a second reflection interface 9, that is, a second reflection interface 9 capable of passing the laser beam incident by passing through the first reflection interface 8 and capable of diffracting at least several partial beams so as to reduce these convergence.
