Bone plates and methods of using the bone plates for adjoining bone fragments are disclosed herein. A device (100) for adjoining at least two bone fragments includes a delivery catheter (110) having an elongated shaft (101) with a proximal end (102), a distal end (104), and a longitudinal axis therebetween, wherein the delivery catheter (110) has an inner void (113) for passage of at least one filling material and an inner lumen for passage of a light source (150); a conformable bone plate (103) releasably engaging the distal end of the delivery catheter (104), wherein the conformable bone plate (103) has a first surface (133), a second surface (143), a proximal end (123), and a distal end and at least one aperture extending from the first surface (133) to the second surface (143) for receiving a fastener (163); and an adapter releasably engaging the proximal end of the delivery catheter (102) for receiving the light source (150) and a delivery system housing the at least one filling material.
