The technique for thoracoplasty comprises the paravertebral access, the resection of the posterior-lateral segments of the ribs I-V and the complete resection of the ribs I-II, the extrapleural apicolysis, the compression of the cavern, and the fixation of the apex of lung. Prior to the fixation of the apex of lung, semi-purse-string sutures are applied at the site of the cavern and in the paravertebral space. The punctures are performed at the layer of the intercostal VI, through which the free ends of the ligatures are passed and tied up allowing for pulling the apex of the lung towards the rib VI. Then extrapleural cavity is tightly tamponed with the collagenous biopolymer. |
