PROBLEM TO BE SOLVED: To provide a system for guiding catheter insertion, capable of indicating an advance direction of a catheter in a blood vessel to an operator and avoiding a contact between a catheter distal end and a part of a stenosed blood vessel by displaying an accurate positional relationship between the catheter distal end and the part of a stenosed blood vessel; and a medical image diagnostic apparatus equipped with the same system. SOLUTION: This catheter insertion guide system includes: a blood vessel core wire extraction means 14 for extracting a blood vessel and a blood vessel core wire based on a three-dimensional image related to a subject; a calibration means 15 for positioning a location of the subject and a location of the three-dimensional image based on catheter position information; an image positioning means 12 positioning the three-dimensional image and a fluoroscopic image of the object blood vessel based on information or the like from the blood vessel core wire extraction means 14 and the calibration means 15 and calculating a direction where a distance from the blood vessel core wire to the blood vessel internal wall becomes a minimum in the object blood vessel; and an image generation means 16 for generating a guide display image of the catheter based on the information from the image positioning means.
