PROBLEM TO BE SOLVED: To provide an image reader capable of stably and accurately moving a carriage with simple constitution, and an image forming apparatus equipped therewith. SOLUTION: The image reader 5 includes: first and second carriages 71 and 72 which are supported movably in a scanning direction; a first driving wire 26a which connects the carriages 71 and 72; a driving pulley 25 round which the driving wire 26a is wound; a driving shaft 231 to which the driving pulley 25 is fixed; and a first driven pulley 271 over which the first driving wire 26a extended from the driving pulley 25 is laid. The first driving wire 26a is driven through the first driven pulley 271 by rotation of the driving pulley 25, so as to move the first and the second carriages 71 and 72, whereby the image of an original is read by making reflected light from a surface of the original enter an image sensor 74 while scanning the surface of the original. The first driving wire 26a is wound round the driving pulley 25 with predetermined width in a driving shaft direction, and the first driven pulley 271 is juxtaposed with the driving pulley 25 in the scanning direction nearly in the center of the predetermined width.
