A device for monitoring patient usage of a medicament inhaler (4) is disclosed. The device includes a housing (3), an optical dose counter (13) and an electronics control module. The housing (3) is releasably attachable to, or around, the medicament inhaler (4). The optical dose counter (13) is associated with the housing (3). The counter (13) is adapted to determine when a dose of medicament has been dispensed by transmitting optical signals through the medicament inhaler (4). The electronics control module and/or the optical dose counter is adapted to monitor and/or store information relating to patient usage of the medicament inhaler. The arrangement and construction is such that the device is attachable to the medicament inhaler (4), and subsequently operable, without any modifications required to be made to the medicament inhaler (4).
