A business form or intermediate 10 consists of a single ply pressure seal form that permits extraction of one or more inserts 12 die cut from and contiguous with the single ply. In one embodiment the business form or intermediate comprises two panels 14,16; an insert 12 which may be multi-panelled and a removable pull tab 32. To form the mailer the insert is folded up to overlie panel 16 and panel 16 is then folded up to overlie panel 14 with lines of pressure sensitive adhesive 24,26 engaging to secure the mailer. The pull tab is then enclosed within the interior of the mailer. Removal of the pull tab releases the insert and creates a single opening to permit its removal. In other embodiments a second pull tab may also be present which overlies the first pull tab and which, when removed, exposes or removes the first pull tab. |
