A delivery slip is provided to grant acceptable characteristic to be kept in a first sheet detachable from a second sheet and simply cope with although alteration lies in information amount. A delivery slip(10) comprises a first sheet(11) in which printing is accomplished on both sides of a first sheet base(11A); and a second sheet(12) in which printing(P2) is accomplished on the other side and an adhesive layer(12B) is installed on one side of a second sheet base(12A). The first sheet is temporally bonded to the second sheet through a release treated layer(11B) with folded state. The first sheet is kept between the second sheet and a substrate(W) by adhering adhesive layer on the substrate exposed on the surface of the first sheet. |
