PROBLEM TO BE SOLVED: To provide an image forming device, capable of determining the cause of stripe-like image quality failure on an early stage. SOLUTION: The image forming device 10 comprises a high-voltage power source 21 for generating a charging voltage; a charger 24 for charging a photoreceptor 25 using the charging voltage; a laser diode 23 for exposing the photoreceptor 25 charged by the charger 24 to form an electrostatic latent image; a test image formation control part for driving the laser diode 23 so as to form a predetermined test image in each of a first state having both application of ac component voltage to the charger 24 and exposing, a second state having application of ac component voltage to the charger 24 but having no exposing, a third state having no application of ac component voltage to the charger 23 but having exposing, and a forth state having neither application of ac component voltage to the charger 24 nor exposing; a density sensor 12 for detecting the density of each test image; and a failure position determination part for determining a failure position based on a detection result of the density sensor 12.
