<P>PROBLEM TO BE SOLVED: To provide an image reader which uses a small number of light sources, has a compact illumination optical system, and excels in illuminance distribution, and an image forming apparatus using the image reader. <P>SOLUTION: The image reader 10 includes: a light source unit 8 which emits light that is illuminating light to illuminate an original surface 6a; the illumination optical system which has a light transmission body 2 constituted of a plurality of reflectors 2a to 2d and having light condensing action, and reflectors 3 and 4, and condenses the light emitted from the light source unit 8 by the light transmission body 2 so as to illuminate a reading object area 6a1 having length and width on the original surface 6a by reflecting the condensed light by the reflectors 3 and 4; an imaging optical system which forms an image with light reflected from the original surface 6a; a sensor which is provided at the imaging part of the imaging optical system and reads the image of the original.
