A polyethylene form sheet having anti-static function and manufactured with a laminating process is provided to reduce manufacturing cost by laminating a conductive polyethylene sheet on both sides of the polyethylene form sheet. A polyethylene form sheet having anti-static function and manufactured with a laminating process includes a form sheet used in an internal layer and an anti-static sheet used in an outer layer. The anti-static sheet used in the outer layer comprises the following steps of: manufacturing a polymer resin; manufacturing an extrusion sheet; and welding the anti-static polyethylene sheet manufactured in the polyethylene form sheet.
