PROBLEM TO BE SOLVED: To provide a slip with a card that can be inexpensively manufactured and capable of imparting a superior counterfeiting prevention function. SOLUTION: A card piece 50 is formed surrounded by a cutout line 51 comprising perforated lines in a region of a single piece slip 40, and minute perforated lines 53 comprising minute cut parts 53a and minute uncut parts 53b are included in at least one side of the cutout line 51 of the card piece 50.
