<p><P>PROBLEM TO BE SOLVED: To raise reliability of answer contents concerning an information collection system and an information collection method. <P>SOLUTION: An information collection server for collecting prescribed information from an information acquisition candidate through an information input form is provided with: a medium generation part 105 for generating a medium in which the storage place information of the information input form is written by applying ID to uniquely identify the medium; and an information input form processing part 101 for transmitting the information input form to a client when receiving the acquisition request of the information input form. Also, the client is provided with: an acquisition part 203 for acquiring the storage place information of the information input form written in the storage place information storage medium; a communication control part 206 for transmitting the storage place information of the acquired information input form to the information collection server; a display part 205 for displaying the information input form transmitted from the information input form processing part 101; and an operation part 204 for inputting the prescribed information to the information input form displayed on the display part 205. <P>COPYRIGHT: (C)2009,JPO&INPIT</p> |
