PROBLEM TO BE SOLVED: To provide an apparatus for forming a polyurethane foam, which can prevent carbon dioxide from gasification during feeding. SOLUTION: The apparatus comprises a heater 21 which heats a liquefied carbon dioxide vessel 20 with a syphon tube attached, pressurizing the inside of the vessel to near 8 MPa; first piping L11 of delivering liquefied carbon dioxide in the vessel 20; second piping L12 having 5 MPa inside pressure to feed the liquefied carbon dioxide: and flow regulation vulves 15, 26 and 27 connected to the terminal edge side of the piping L11 and to the starting edge side of the piping L12, which regulate a flow rate of the liquefied carbon dioxide flowing in the piping L12 so that an inside pressure of the piping L12 may become about 5 MPa.
