PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of attaching audio data, video data, etc., together without any assistance by an external computer when transmitting image data etc., read out of a document. SOLUTION: The image forming apparatus includes: a conversion means of converting the image data of the document obtained from an image read means into an image file in a specified format such that audio data and video data can be attached; an acquisition means of acquiring at least one of audio data and video data stored in an external server as data to be transmitted at user's choice; and a transmission means of associating the at least one of the audio data and video data acquired by the acquisition means to the image file and e-mailing information about the image file to a transmission destination.
