PURPOSE:To execute stably recording and reproducing operations, and also to obtain an optical memory card having a high recording density by providing a guide groove for designating a recording place of information, and recording in advance an address signal in the guide groove. CONSTITUTION:A photoresist is applied onto a glass plate by a spin coat, and thereafter, a guide groove forming position is exposed by a condensing laser beam, and an address signal is recorded. That is to say, when forming a guide groove 23 of an address signal recording part, the laser beam is modulated by the address signal and the address signal is recorded as existence of the groove, and by developing it, the guide groove can be formed and a groove original edition is made. Subsequently, a stamper is made by means of nickel electroforming, and by using it, the groove is transferred to a plastic material such as an acryl resin, etc., and a transparent base substance 25 of an optical memory card is obtained. Next, a recording thin film 26 is formed, and a heat insulating source 27 and a protective layer 28 of an acryl compound cure resin, etc. are formed.
