A medical bed having an exercise unit is provided to maintain and promote rehabilitation and health of a patient by enabling the patient to take a leg exercise easily using the exercise unit installed on the bed. A medical bed having an exercise unit includes: a bed frame(10); a guide bar(23) installed on one side of the lower end that a space part is formed on a lower and center part of the bed frame; a slider(24) moving by a belt along the guide bar; a rack gear installed on the other side of the lower end that the space part is fixed; and a connection slider having a pinion gear with a shaft hole formed on the center to be rotated to engage with the rack gear. One side of the connection slider is guided by the rack gear, and the other side is fixed to the slider. A pivot shaft of leg exercise equipment(30) equipped with foot supporters(40) is connected to the shaft hole of the pinion gear installed in a bearing in the connection slider.
