<P>PROBLEM TO BE SOLVED: To provide an application reception management system and an electronic business form, for managing applications in detail according to a contract business procedure when receiving various applications with an electronic means. <P>SOLUTION: In the application reception management system, an application reception device is connected with an application terminal device via a network, the application reception device has a previously registered electronic business form, and the application terminal device acquires the electronic business for application. The electronic business form includes an input form used for inputting predetermined application data, an additional material related to the application, version management data for the input form, date data for identifying the application time, and a log information creation means for recording log information including the version management data for the additional material and the date data for identifying the application time.
