An apparatus (10) to transfer power from rotary power to reciprocating power includes an odd numbered muiti-lobed rotating cam (12) that acts on a follower, which, in turn, causes or acts in response to reciprocation of pistons (36, 38). The follower (34) includes offset head portions (55, 57) and the cylinders (12, 14), in turn are actually offset in the direction of the head portions. This reduces friction and improves efficiency. Further, the cylinders can each include two inlets (104,104a, 106,106a) and two outlets (108,108a, 110, 110a) so that each cylinder functions when the pistons (36, 38) move in either direction within the cylinders.
