PROBLEM TO BE SOLVED: To provide a form for a tunnel entrance, which is applicable to different topographic features, facilitates setting thereof, and contributes to a shortened construction period and reduced construction costs. SOLUTION: The form for the tunnel entrance 1 is formed of: an inner form 2 having a surface curvedly formed so as to follow an internal surface of the tunnel entrance 1; an outer form 3 having a surface curvedly formed so as to follow an external surface of the tunnel entrance 1; and an end form blocking a tunnel entrance-side opening section between the inner form 2 and the outer form 3. Herein the form for the tunnel entrance has winding thickness holding members 20 arranged between the inner form 2 and the outer form 3, for supporting the outer form. The winding thickness holding members 20 are set between the inner form and the outer form, and when concrete is poured between the forms, they function to prevent deformation of the external form and to secure a predetermined winding thickness.
