PROBLEM TO BE SOLVED: To simply and certainly prevent an influence of return light reflected by a composition surface (including a lens surface) of two or more optical waveguide components. SOLUTION: The slab optical waveguide substrate includes: a two or more input channel optical waveguides 4 arranged parallel to each other; two or more collimator lenses 5 arranged so that their optical axes are parallel to each other; two or more recessed parts 6 for mounting other optical waveguide components; a slab optical waveguide 7 provided between the two or more recessed parts 6; two or more condenser lenses 8 arranged so that their optical axes are parallel to each other; and two or more output channel optical waveguides 9 arranged parallel to each other.
