PROBLEM TO BE SOLVED: To enable reproduction of an address with least possible overhead and with high reliability in an optical disk.

SOLUTION: The side faces of a plurality of unit section portions arrayed along track grooves has the side faces displacing periodically along the track grooves and express the sub-information allocated to each unit section portion by the shape allocated to each unit section portion. Each of the unit section portions has a first side face displacement pattern regulated so as to be relatively steep in the rise of the signal waveform and to be relatively gentle in the fall or the second side face displacement pattern regulated so as to be relatively gentle in the rise of the signal waveform and to be relatively steep in the fall. The identification mark for each unit section portion is arranged at the top of the corresponding unit section portion. The identification mark has a side face displacement pattern discriminated from the first and second side face displacement patterns and expresses the same information assignment as the sub-information expressed by the shape allocated to the corresponding unit section portion.
