<p><P>PROBLEM TO BE SOLVED: To solve the problem that conventional slips for consumers which are constituted of a plurality of sheets have a difficulty in recognizing on which paper an authentication column is present, and therefore, a concerned person such as the consumer is difficult to spontaneously perform an authentication procedure on the paper, and that there is a tendency that the authentication procedure is committed by the guiding on the side of an entrepreneur to make legal troubles frequently. <P>SOLUTION: By enabling a name column, the authentication column for the name column, and a name person designating region to be checked from the top of the upper sheet of paper, on which paper the person concerned can perform the authentication process can easily be understood provided that a peculiar relationship between the name column and the authentication column is ensured on papers which become each document.
