PROBLEM TO BE SOLVED: To provide a substrate for hologram recording medium formed of a polycarbonate resin composition jointly having low birefringence, high rigidity, low moisture permeability and low oxygen permeability, and to provide a hologram recording medium. SOLUTION: The hologram recording medium is composed of: a light transmissive substrate 1; a substrate 2; and a recording layer which is formed between the substrate 1 and the substrate 2 and contains a three-dimensional cross-linked polymer matrix, a radical polymerizable compound and a light radical polymerization initiator. In the hologram recording medium, the light trnsmissive substrate 1 and the substrate 2 are each composed of a resin composition obtained by adding a higher fatty acid ester and a phosphorous based stabilizer to a polycarbonate resin composed of a cyclohexyl group-containing carbonate constitutional unit [A] and a carbonate constitutional unit having a single bond, and, a 1 to 15C alkylene group, or a 1 to 20C aromatic group-containing alkylene group or a unit obtained by boning substituted or nonsubstituted two phenyl groups bonded by oxygen atoms; wherein the ratio of [A] is 80 to 100 mol%.
