PROBLEM TO BE SOLVED: To provide a duplicating form with a label consisting of two forms, the first form and the second form which have nearly the same area and shape, and to provide a booklet employing thereof. SOLUTION: In the duplicating form with the label consisting of two sheets of form, on which information is written at least by an electron pen having a writing part for viewably writing information and a picking-up part for reading dot pattern printed on an information carrying surface, the first sheet of form and the second sheet of form of the duplicating form with the label consisting of the two sheets is the form having one and the same shape with nearly one and the same area bound together at one end of every one form under the condition that the first sheet of form is a laminate of the base paper, self-adhesive layer and releasing paper stacked in the order, a duplicating structure is formed in the contact surface between the first sheet of form and the second sheet of form and, on at least either the expressional surface of the base paper of the first sheet of form or that of the base paper of the second sheet of form, the dot pattern is printed, and a booklet employing the duplicating form.
