<P>PROBLEM TO BE SOLVED: To provide a seal mechanism which is not restrained by a seal maintaining performance and by the ability to maintain the integrity when a device is moved inside a portal and prevents the loss of fluids (for example, the loss of a blow gas) caused by grooves generated around the device by movements in an extreme region of the device of a small diameter in the portal member. <P>SOLUTION: A surgical access apparatus includes a housing member, a portal member extending from the housing member to define the longitudinal axis, and a position adjusting mechanism provided relative to the housing member. The housing member and the portal member define a longitudinal passage which has dimensions to enable the passage of an elongated object and passes the inside.
