Apparatus for and method of forming concrete and transferring loads between concrete slabs

A plate for transferring a load between adjacent concrete slabs is disclosed. The plate is cast in place to form a joint between first and second slabs. The plate has a coating on one portion (320), and the first slab contacts only the coating while the second slab contacts only the uncoated region (315) of the plate. The first slab is able to move relative to the plate.
