Apparatus for and method of forming concrete and transferring loads between concrete slabs

Methods and apparatus for forming concrete are disclosed, where a plate is left embedded in the concrete to discourage vertical movement between adjacent concrete slabs. The plate (300) has a first coated portion (320) and a second portion (315), and a form (200) is provided with a slot (260) configured to closely receive the second portion. The second portion is inserted into the slot and the form is positioned to receive the concrete. Concrete is poured on the form and the first portion, and then cured. The form is removed from the cured concrete, while the plate remains in place.
