An extrusion forging product manufacturing method using a back pressure type cold forming device is provided to perform forging and sizing at the same time by applying the back pressure in the direction opposite to the extruding direction in the extrusion forging. An extrusion forging product manufacturing method using a back pressure type cold forming device comprises an upsetting step(S100) for pressurizing the S25C material to reduce the thickness; a forging step(S200) for seating the thinned material in a forging mold and pressurizing it to form a semi-product; a piercing step(S300) for punching the inside center of the semi-product; and a back pressure molding step(S400) for forming teeth on the lower circumference of the semi-product by applying the punch load and the back pressure of opposite direction to the punch load to the semi-product at the same time, and extruding and sizing the semi-product. Cold forging is applied in the forging step and back pressure molding step.
